A claim declined, a complaint rejected, an application refused. The decision-maker owes you the reasons. This asks for them, by the rule that says so.

  2. The findings of fact the decision rests on, and the documents each finding was based on.
  3. The name and position of the person who made it, and the date.
  4. Every avenue of review open to me, and the time limit on each.
