Dear Prosecutor, and the officer with carriage of this matter,

I am the accused in [their reference], listed on [the date they received it]. I act in person and represent myself.

[the person] do not act for me. They hold no authority from me. I have terminated the retainer and confirmed to them in writing that they do not act for me. They cannot act, receive disclosure, or correspond on my behalf without my written authority, and I give none.

I therefore require that:

  1. All correspondence, disclosure and service in this matter be directed to me personally, at this email address; and
  2. My matter, and my personal and health information, not be discussed with or disclosed to [the person], or any person who does not hold my current written authority.
