Maladministration. To the Ombudsman.

A WA department did something its own record shows it should not have. Three failures, each on a document, and one ask.

Subject: Complaint of maladministration — [the agency][your full name]

Dear Ombudsman,

I complain of maladministration by [the agency] in its handling of a matter concerning me: [what happened].

The specific failures

  • A record was made that says one thing. The source document says another. The record was not checked against the source, circulated for months, and was acted on.
  • A conflict of interest existed when the matter was taken on. I am aware of no record that it was identified or declared.
  • The department later denied a finding it had itself made. I hold the finding, and have provided it.
