Report a public officer to the CCC

Serious misconduct by a police officer or public servant goes to the Corruption and Crime Commission. Its form has eight boxes. Fill them in this order.

Subject: Report of serious misconduct — [the person], [the agency]

Allegation

Serious misconduct by [the person] of [the agency]: [what happened].

Date

[the date they received it].

Location

Where it happened. Station, office, address.

Agency

[the agency].

Officer involved

[the person], with rank and registered number if known.

What happened

  1. One fact per number. What was said or done, by whom, when.
  2. What the officer’s own record says. Quote it.
  3. What the officer did with the record. Quote that too.
  4. Any prior finding by another body on the same matter, with its date.
  5. Any connection between the officer and a person on the other side.

Witnesses

Names, or “available on request”. The recorded interview and the running sheet are witnesses too; say they exist.
